There is a slight taper to the plastic cartridges so that they can be 
removed from the mold after manufacturing. If the lubricant is not spread 
around properly before filling with insulin, it is possible for the 
plunger to bind near the end of the cycle causing the problem you observed.

Before filling the syringe, run the plunger up and down a couple of times 
while twisting it. This will adequately swish the lubricant around. Don't 
do it more as you can actually rub the stuff off.
