Once a clinical decision has been made that you need a pump, then the
PCT cannot refuse to fund.  You should not need to apply any pressure,
but in reality you may need to do a bit of chasing.  Your consultant
needs to write to your PCT for funding.  If you don't hear from the PCT,
ring up the Commissioning Manager and ask why you haven't heard.  If
they say / write that no funding is available, tell them you know about
their obligations to NICE guidance.  If they still insist there is no
money, contact John Davis on email @ redacted
