 Glycaemic instability can sometimes be a consequence of not having enough basal
insulin. This can trigger a temporary reduced insulin sensitivity leading to a
reduced response to a bolus. Maybe (for whatever reason) your basal requirements
have gone up a bit over the period before breakfast. You might try increasing
your basal over the period of 2-3 hours before breakfast.
