Are you in the US?  Go to your states web site, or do a search to find out
who your states Insurance Commissioner is.  Write a letter complaining to
your medical director of your insurance, and Carbon Copy it to your
Insurance Commissioner, with a note telling the Commissioner of your
difficulties.  Make sure you put on the bottom of the letter that you are
cc'ing it to the IC.  It might take a month or so, but I guarantee you will
hear from both sides with a solution.  All it takes is a squeaky wheel to
the Higher Powers and the Insurance Co will be as sweet as pie.....  I know.
It worked for me.
