Yes this happened many times until I learned that the insulin can not be
allowed to reach room temperature or the 30 day expiration starts.

Usually this happened because I picked up my insulin at the pharmacy and
kept it on my person until later that day when I was home.

Now of course I keep it refrigerated until ready for use.
