You still need to account for all the carbs you eat regardless if you have
insulin on board. The IOB may only have to do with a previous carb load or a
correction.

What I've been taught is that a carb bolus needs to be reduced if there's
anything else on board
I agree, it sounds counterintuitive, but that's what I've been taught
