Tim,

I also received an invoice with my pump and I was covered 100%.  I called a
MM Rep. right away and he apologized numerous times and mentioned that there
was an error in the bill, that there was supposed to be a statement at the
end saying that the insurance company(s) are being billed...do not submit
payment.  I pretty much figured this was the problem.  He also mentioned
that they include the invoice listing the prices for pump and accessories so
that you can submit that for insurance purposes (if you plan on getting
coverage for the pump).
