If you always pack a 45 gram lunch and, assuming she always eats the entire
lunch, the bolus for lunch is always the same.  The question then is any
adjustment due to high or low bg at lunch time.  You could put guidelines
on a notecard in her lunch for figuring any adjustments needed.  Have you
asked her if she thinks she is ready to try this on her own.
Christmas holidays are coming up pretty soon.  Do an experiment over her break
and let her make all her bolus decisions for lunch.  See how she does.  It of
course is better if she underestimates her needs, than overestimating and
going hypo.  Then, make sure she tests 2 hours after lunch. (Is that possible
at school?)
It sounds like she has a good handle on things.  Ask her how she feels about
you coming to school every day and if she is ready to try it on her own.
Sounds like you are doing a great job with her!
