I keep a current medical history and medication sheet on my PDA memo pad
which is always with me.
Medications (name, dosage, how often) are listed by, system and occasional
p.r.n. use. (when needed) plus daily vitamins.  Also date when started and
when discontinued.

It's easy to update, on the spot, and print out on the computer for Dr.
appointments etc.  For us "seniors" our vital information is right handy
when memory fails.
