She must fight the battle. The request is unreasonable and has 
nothing to do with the LETTER OF NECESSITY provided by the doc. It is 
a smoke screen to delay payment. The litany is the same. Appeal, 
complain, escalate.... etc..... insurance commissioner, doctor,
