[IP] sils

Use only about a 10 degree angle to insert, 30 is way to steep. Pinch the
skin hard to provide another source of pain during insertion. Once the
needle is in, push the sil home almost parallel to the skin then fasten it
down and remove the inserting needle. You can ice the skin first to reduce
some pain but a nice hard pinch does it too.  Once the inserting needle
gets into muscle it hurts like h..ll.
