You can find records on medical device failure at 
http://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfMAUDE/search.CFM

Enter Manufacturer MiniMed, Event Type Malfunction, and Product Code 
LZG.  Note that reports take some time to be entered so the data aren't 
really up to date.
