Send them another copy of the bill, WITH a copy of that letter. You have
it in WRITING! 

Could be some not-too-smart and/or lazy clerk just didn't look at your
file to see if the pump had been approved. That happened to me when I
was trying to get my pump -- they approved the pump, but not the
supplies, and turns out that the supply approval should have been
automatic, but no one looked to see!
