What a frustrating experience!  You might think about contacting the 
grievances/appeals department of your insurance company and let them know the 
trouble you are having.  Sometimes the people in those departments can be 
very helpful and help you to resolve situations more quickly.  I would also 
back up a call with a letter to the same department so you and the insurance 
co. have it all in writing.
