Failure to refer under the circumstances you describe is a breech of 
his/her hippocratic oath and borders on malpractice since she is in 
need of a specialist. She should complain to the HMO and the medical 
certifying board for the state she lives in about this doctor. She 
will probably get quicker results by making her intention to do this 
known to her PCP in a letter or phone call and giving him/her one 
more chance to do the right thing.
