Hi Erin,  I just finally recieved my Paradigm on the 30th of June.  Mini Med
will want  a referral letter from your Dr. sent to your insurance company,
3 months of your blood Sugar readings ( a copy of your log) ,  your last 3
Hb A1c results.   Please  stay in e-mail/phone  contact with your MiniMed
Representative and the MiniMed insurance handler  they were a great help to
getting the correct forms from my Doctor and I hope you get speedy results
:)  I had to wait for my Dr to send the referral form to my insurance.  I
had applied  in April but believe me it was worth the wait:)
I will begin with Training on 7/10 (Wed) with Saline Solution in the pump
then i will need to change my own infusion set on Saturday the 13th. then on
Monday the 15th the pump trainer will come and help me to fill my next
reservoir with insulin.
