Lots of people have bubble problems for lots of reasons. One thing to 
consider is that the solubility of air in water increases as the 
temperature decreases. Therefore, your problem could be caused by the 
solution slowly out gassing while in the reservoir. To minimize this you 
might try bring the insulin to room temperature for a realtively long time 
(overnight) before filling the reservoir. Also be sure that you are not 
storing the vial pressurized with extra air.
