The amount of insulin I can get from my insurance has pretty much always
been based on what my dosage is which should be written right on the
prescription; thus using 30 units/day of Humalog I need 1 bottle/month
(1000 units).  Actually I get 2 bottles since I can use up to 35 units a
day.

When I used ultralente/humalog I used to get 2 bottles of the UL and one
bottle of the H, again calculated on my monthly dosages (X3 if
prescriptions are gotten every 90 days).
