Often times, portability of health care depends on who carries the insurance. 
 I once worked for the University of Colorado who is self-insured.  Because 
of their self insured status, the state insurance commission said that they 
were not required to cover any preexisting conditions---even the meds for it. 
 This may be different now because of changes in federal laws, but guaranteed 
you'll have to check into things.
