Consumer Information on Voluntary Recall of Certain TRUEbalance and
TRUEtrack Blood Glucose Meters

The company has determined that certain isolated TRUEbalance and TRUEtrack
Blood Glucose Meters have an incorrect factory-set unit of measure. If a
consumer did not notice the incorrect unit of measure, it is possible that
the meter result could be read as a lower or higher than expected blood
glucose result.

To check if you have an affected meter:

If you received your meter in the United States or in a US Territory, please
enter your serial number below to see if you have an affected meter:

Please locate the serial number on the back of your meter just above the
battery door. See below for instructions on locating your serial number.

You can also call Stericycle toll-free at 1-866-236-4518 to determine if you
have an affected meter.

If you received your meter outside of the United States, please enter your
serial number below to see if you have an affected meter:

Please locate the serial number on the back of your meter just above the
battery door. See below for instructions on locating your serial number.

For your TRUEbalance meter, please submit your serial number beginning with
the letter B and the 7-digits after.

For your TRUEtrack meter, please submit your serial number beginning with
the letter E and the 7-digits after OR if you do NOT have a letter E before
the number, please submit the 7-digits of the serial number.
