There are a number of log sheets and log books on the LOGBOOK page of 
the insulin pumpers web site, including one that is a post script 
page that can be taken to any copy shop. It will make a number of 
compact logbooks just for pumper from a dozen 8 1/2 x11 sheets after 
it is cut and stapled.
