This is another of those absolutely YMMV things, and you are going to
have to experiment to find out how YOUR body works.

My own experience is to bolus for carbs with the arrival of the pizza
and then to bolus an additional 2 or 3 units 2 hours later to cover the
protein and fat. It usually works.

Some people use a temporary bolus -- amount of time varies, depending on
how you metabolize food. 

I'm sure you'll get a lot of answers to this -- read 'em all, and
they'll give you some really good ideas on where to start, but you're
still going to have to find out for yourself!!!
