I once had one come out Z shaped!  Two 90degree PLUS kinks. I've had a
few others with only one kink.

I think this happens when the set comes partially out, and then is
shoved back in again by movement. 

Most of the time, I've known the kink was there and been able to replace
the set promptly because it felt funny, but sometimes you don't feel
anything at all.
