What a booger! :) 

Now this is going to sound cruel, but get one of those (HUGE!!!)
gourmet meat-baster syringes (if you don't have one, just use a
regular syringe, although the effect won't be the same) and take
that and a tourniquet in his room with you to do his bg. As
you're tying on the tourniquet (not too tight, just enough to get
his attention), non-chalantly tell him you're just going to need
a little blood for the bg test and could he please pump his fist
a couple of times for you. If that doesn't get his attention,
nothing will! ;)
