I am on the 511 and using the sof-set and this does occasionally happen to
me. I sometimes get a site that is more sensitive than at other times.  By
that I mean it stings a little on insertion and usually looks like yours
when I remove it.  With me, this always happens in the same approximate area
of my abdomen.  My spot is just below and to the right of my BB about 1.5"
in diameter.  I try to avoid it if at all possible.  Let me know if you find
this to be true about your sites.
