I had this problem when I first started on the pump and was using the quick sets.  I switched to the silouettes which uses less than a 90 degree angle that the quick set uses (I think between 20 and 40 degrees, you can choose with the sil-serter)...Haven't had a problem since...I think the cannula bends when it hits muscle.
