It is an "old fashion" way of saying that control is difficult. This 
is usually do to the fact that the long smooth curve of NPH or Lente 
insulin does not match the basal requirements of the person taking 
the insulin. The result is often the "roller coaster" ride of highs 
and lows as the person's insulin needs go up and down during the day. 
Other than MDI with Humalog with lots of the "multiple" part :-) 
there probably is no better way to solve this problem other than a 
pump or a working pancreas.
