Julie,
I have a living will. You can do it yourself and it would have to be
witnessed by 2 people and notarized. My lawyer did mine for me. we addressed
everything from medical care, life support, CPR where I am to be taken care
of if I live through a  stroke and even if I were to need kidney dialysis.
Issues such as feeding tubes/nutrition were addressed should I ever not be
able to communicate to others. Even organ donation is written down. As well
as how my body is to be taken care of. I believe this frees the family of
making some pretty difficult decisions.
There is some legal home programs which will guide you through this and once
the questions are answered you can print it out. All my doctors have a copy
as well as my hubby and my children and the hospital.
I think this is a wise thing to do.
