. Refills will not be authorized with this coupon.
. Please remove the identification number from the patient profile after
claim is processed.
. Date of birth and prescriber D.E.A. number are both required.
. For assistance in filing this claim, please call the Anthem Help Desk,
1-866-291-1621.
Claim for product dispensed pursuant to this card shall be submitted to
Anthem ON L Y for reimbursement and not to Medicare, Medicaid or any third
party payer. Any use of this card other than as indicated constitutes fraud.
Failure to produce to Bayer, on request, invoices and other information
demonstrating all conditions have been met, will void all coupons submitted

I don't know where you live, but when I called, they said that they would
send me a coupon for free meter. They did not ask my dr's name or phone or
my ins info!

Not quite free. They will contact your Doctor and get a Rx for the Breeze
Glucometer. That will switch you from what ever you are using to the Breese.

I was watching a Cubs baseball game on cable (WGN), and Bayer advertised a
free Breeze Glucometer. I called the toll free number 1-800-554-9393,
answered a few questions, and was told that the meter would be mailed to me.
This is the new cartridge type meter. btw, the Cubs lost. . .
