In order to insure positive air pressure inside the vial of insulin, you 
need to inject at least as much air into the vial as insulin you will be
taking out before you attempt to fill your new cartridge.
 It is better to open the vial to air by inserting a pump reservoir needle to
equalize the pressure of a vacuum. You may still need to inject a little air to
get some positive pressure, but in the end it will mean less air dissolved in
the insulin to be released in the cartridge later on.
