Since they used your credit card to defraud you, you have lots of protection
under the law.  The first thing to do is go to your card-issuing bank and
insist that they initiate a chargeback inquiry.  They have to do that at
your request.  At that point the charge is reversed from your account.
Then, depending on which credit card network you are using (Visa, MC, Novus,
JCB, etc.) the pharmacy has some small number of days to respond to the
inquiry, or the reversal remains permanent.  The pharmacy will have to
either produce documentation that you authorized the charge in question, and
that the insurance company denied the claim, or they'll have to eat the
chargeback.  If you have documentation that the claim was paid by the
insurance company, FAX it to the card-issuing bank.

If you don't get satisfaction there, you can complain directly to the
network that issued the card (Visa, MC, Novus, JCB)
